Information Statement Instructions (UCC5)

Who Can File Here

  • Anyone wishing to file a UCC5 Information Statement which has not previously been filed with the Tennessee Secretary of State.

    Pursuant to T.C.A. § 39-17-117, it is a Class E felony for any person to knowingly prepare, sign, or file any lien or other document with the intent to encumber any real or personal property when such person has no reasonable basis or any legal cause to place such lien or encumbrance on such real or personal property.

  • The file number of the related UCC1 Financing Statement must be provided in order to process a UCC5 Information Statement. Search the UCC database to retrieve the required information prior to starting the filing wizard.
  • An Information Statement can be filed and paid for online (credit/debit card or e-check required). If not paid online, a check for the appropriate filing fee must be included with the document when mailed in for processing.

  • A service fee is charged if filing fees are paid online by credit card, debit card or e-check.
  • Pursuant to T.C.A. §47-29-102 a handling charge of $30.00 or the written amount of the item, whichever is less, will be charged on returned paper checks and electronic checks.
